Community Health Improvement Plan (CHIP)
The CHIP document outlines information on the process used to select the two priority areas (obesity and behavioral health) for the community to focus on from 2018-2022. The CHIP provides information on the priorities and the philosophy behind the plan. This document is meant to be the basis for action plans. The action plans will be living documents that will be updated to demonstrate progress and change to reflect growth, barriers, and innovations. Chronic Health Conditions and Access to Care in Pueblo
The purpose of this report is to identify leading chronic disease and access to care issues in Pueblo County in order to be more successful in current and future community public health efforts. This report was completed in 2016.
Pueblo County Food System Assessment
The Food System Assessment looked at local and regional food systems of Pueblo County to provide more safe, fresh, and healthy foods to improve food access, food security, and health of all Pueblo residents.
