Community Health Improvement Plan (CHIP)

The CHIP document outlines information on the process used to select the two priority areas (obesity and behavioral health) for the community to focus on from 2018-2022. The CHIP provides information on the priorities and the philosophy behind the plan. This document is meant to be the basis for action plans. The action plans will be living documents that will be updated to demonstrate progress and change to reflect growth, barriers, and innovations. Behavioral Health in Pueblo County 2017

The purpose of this report is to give data and information on behavioral health issues, including mental health and substance abuse in Pueblo County. The information provides available data on the topics along with information on health disparities.  

 

Obesity in Pueblo County 2017

The purpose of this report is to give data and information on obesity and related conditions such as heart disease, diabetes and more in Pueblo County. The information provides available data on the topics along with information on health disparities.  

 

Pueblo County Health Status Report 2016

The purpose of this report is to give data and information on a variety of topics in Pueblo County including health, economics, education, environment and more. This is the second Pueblo County Status Report and it was created in 2016.

 

Pueblo Community Health Status Report 2011

The purpose of this report is to give data and information on a variety of topics in Pueblo County including health, economics, education, environment and more. This was the first Pueblo County Status Report and it was created in 2011.  

 

Chronic Health Conditions and Access to Care in Pueblo

The purpose of this report is to identify leading chronic disease and access to care issues in Pueblo County in order to be more successful in current and future community public health efforts. This report was completed in 2016.  

 

Pueblo County Food System Assessment

The Food System Assessment looked at local and regional food systems of Pueblo County to provide more safe, fresh, and healthy foods to improve food access, food security, and health of all Pueblo residents.
