The Pueblo Department of Public Health and Environment has seen a surge in inquiries from individuals requesting verification of COVID-19 immunization records in the Colorado Immunization Information System (CIIS). Likely, this is in large part due to the Colorado Comeback Cash Campaign.

Colorado Comeback Cash is a random weekly drawing of $1 Million. One person, aged 18 and older, is drawn each week through July 7, 2021. For ages 12-17 years of age, five $50,000 scholarships will be given each week through random drawings for five weeks. To be eligible, the individual must have had at least one COVID-19 vaccine, and the record must be in CIIS or Veteran’s Assistance (VA) Immunization Records. Individuals can look up their immunization records by visiting ciis.state.co.us/public/Application/PublicPortal, call 877-268-2926, or call their primary care provider.

If Pueblo County residents are not able to access their immunization records by the methods above, they can fill out the confirm COVID-19 vaccination record form. If individuals have misplaced their white COVID-19 vaccine card, they can request a replacement by filling out the request COVID-19 vaccination card form.
