In Colorado, the process for obtaining a handicap placard is standard across the state, but you can complete most of it through your local Pueblo County office or online.
1. Determine Your Eligibility
To qualify, a medical professional must certify that you have a physical impairment that limits your mobility. Common qualifying conditions include:
- Inability to walk 200 feet without resting.
- Need for an assistive device (cane, crutch, wheelchair, etc.).
- Severe respiratory, cardiac, arthritic, or neurological conditions.
- The use of portable oxygen.
2. Complete Form DR 2219
This is the Persons with Disabilities Parking Privileges Application.
- Applicant Section: You fill out your personal information and identify your "Secure and Verifiable ID" on page 1 of the DR 2219 (like your CO Driver's License).
- Witness Section: A third party (not the doctor and not you) must witness your ID information and sign the form on page 1 of the DR 2219.
- Medical Section: Your doctor, Physician Assistant, or Advanced Practice Nurse must fill out the medical certification portion on pages 5 & 6.
- Note: Chiropractors and Physical Therapists can only sign for Temporary (90-day) placards.
- Must have a Colorado ID/Driver License to get any placards besides temporary placards (One Time Issuance)
3. Choose Your Placard Type
Based on your doctor's certification, you will receive one of the following:
- Permanent (Blue): Valid for 3 years; requires a new medical signature every 9 years.
- Extended (Blue): For conditions lasting at least 30 months but not for life.
- Temporary (Red): Valid for 90 days (can be renewed once).
- Remuneration Exempt (Purple): For specific severe disabilities that exempt you from paying at parking meters.
4. Submit the Application
In Pueblo County, you have three ways to submit your completed DR 2219 Parking Privileges Application:

Important Fees & Facts
- Cost: There is no fee for a handicap placard in Colorado.
- Quantity: You may have
- Up to two placards OR
- Two sets of person with disability (PWD) plates OR
- One placard and one set of person with disability (PWD) license plates.
- Display: The placard must hang from your rearview mirror while parked but must be removed while the vehicle is in motion.
- Grace Period: There is no grace period on handicap placards
- Placards are never tied to a specific vehicle and can follow the handicap individual in any vehicle they are in.
