Patients requesting a copy of Medical Records can print the Authorization to Release Information From PDPHE Clinic Form, complete it and bring it to the Pueblo Department of Public Health and Environment.

Guidance talking to your teen about their sexual health

Health Promotion Specialists are available to answer questions, provide training and guidance about talking to teens. For more information call 719-583-4342.
