Monday, Tuesday, Wednesday, and Friday - 8:00 am - 4:30 pm (Closed for lunch 12 - 1 pm)
Thursday - 8 am - 12 pm, International Travel 1:00 - 4:30 pm

Medical Records
Patients requesting a copy of Medical Records can print the Authorization to Release Information From PDPHE Clinic Form, complete it and bring it to the Pueblo Department of Public Health and Environment.
Patients requesting a copy of Medical Records to be sent to the PDPHE Clinic can print the Authorization to Release Information To PDPHE Clinic Form, complete it and bring it to the Pueblo Department of Public Health and Environment.
Immunizations Available
- DTaP (Diptheria & tetanus toxoids & acellular pertussis)
- Hepatitis A (HepA)
- Hepatitis B (HepB)
- Hib (Haemophilus influenzae type b)
- HPV (Human papillomavirus)
- Influenza (Flu)
- Measles/Mumps/Rubella (MMR)
- Meningitis (ACWY)(B)
- Pneumonia (PCV13)(PPSV23)
- Polio (IPV)
- Tetanus/Diphtheria/Pertussis (Tdap)
- Varicella (Chickenpox) (VAR)
- Rabies (upon special order request)
Tuberculosis Skin Testing (PPD) testing is available; appointment is preferred. You must come back 48-72 hours after the test is placed so it can be read.
Travel Immunizations specifically related to travel are available through our full-service International Travel Clinic.
Strict confidentiality is maintained in all services.
Payment
Accept cash, checks, credit cards, or debit cards. Unfortunately, not all HSA cards are recognized at our facility.
The following insurance may be billed for routine adult vaccines:
- Blue Cross/Blue Shield
- United Healthcare (excluding UMR)
- CIGNA
- Medicaid
- Kaiser
Unable to bill Medicare, private HMO plans, and out-of-state PPO plans.
