To report a suspect or confirmed outbreak of COVID-19, complete the COVID-19 Outbreak Report Form and the COVID-19 line list template and send via secure email to [email protected].
In order to complete the line list, begin identifying staff with COVID-19-like symptoms (fever or feels feverish, chills, sweats, runny nose, sore throat, loss of smell and/or taste, headache, cough, wheezing, shortness of breath, difficulty breathing, chest pain, nausea or vomiting, abdominal pain, diarrhea). It is required to place all symptomatic and confirmed positive individuals on the line list.
